How do I make a Task List template?

I have a task list in my public folders. I am setting up this task list for
a small number of people to access and post to in order to keep track of
tasks toward a larger goal. I want, each time a new task in opened, to have
about a 2-3 line header at the top of each task which will help maintain a
standard format for the overall task list.
