How do I make a downloadable form in Frontpage 2002?

I would like to make a booking form that people can download and fill in and
send back to me. Is this possible in Frontpage 2002?

What do you mean; "send back to you"? By mail? By Fax?

If you are hosted on a server that has Front Page
Extensions, you can use a Front Page Form (see help
for "form") and allow your form to be filled out and sent
directly to your email. The result won't look very much
like your on-screen form but it will contain all of the
data provided by the visitor filling out the form.

Front Page Extensions require that you publish (File |
Publish) your site using the "http" method rather
that "ftp" method as FTPing will break the extensions and
render your form useless until you have your host
reinstall the extensions.

If you want people to snail-mail or fax you the filled
out form (in case you want a signed copy on file), have
them fill out the form, print it on their printer and
mail or fax it to you.
