how do i make a contest entry form

I am trying to make a contest entry form for a free draw for a gift basket,
fields i need include name, address, telephone, email, and then some
questions answered using check boxes, is there anyone out there that can help?

Thanks to Suzanne and Jay, these work great (Jay's) if i am emailing a word
document but not so good in print form.

I would have thought it would be just the other way around. All the stuff in
http://word.mvps.org/FAQs/TblsFldsFms/LinesInForms.htm is directed toward
printed forms, as is the EQ field check box I described. If you were doing
an online form, you would use a protected form with check box and text form
fields.
