I am trying to make a contest entry form for a free draw for a gift basket,
fields i need include name, address, telephone, email, and then some
questions answered using check boxes, is there anyone out there that can help?

In addition to these instructions for lines, see the very end of
http://home.earthlink.net/~wordfaqs/Overbar.htm for some helpful tips about
check boxes.

I would have thought it would be just the other way around. All the stuff in
http://word.mvps.org/FAQs/TblsFldsFms/LinesInForms.htm is directed toward
printed forms, as is the EQ field check box I described. If you were doing
an online form, you would use a protected form with check box and text form
fields.
