how do I have a template with check boxes...

Im creating a templete for my work in which I have paragraphs of info. then a
series of checkbox questions then more paragraphs.
I cant figure out how to make it so you can type in the document and do the
check marks without having to "lock the document" each time I want to check
the boxes, then unlock to type? Please help me, I dont know much about this
Im just a student and my boss is demanding of this document!
 
You need to insert text form fields for data entry. Or you can insert
section breaks and protect just the sections that include check boxes. You
need to learn more about protected forms; see
