how do I get my custom contact form published on other computers

I have a custom contact sheet. I transferred it to my employees computer with
a CD, now we cant get outlook to open it!
Also the form does not show up as to a neat little contact sheet icon, ( on
my computer where it was created) yet remains a note with a push pin stuck in
it. It works though! We all have brand new computers with outlook 2003
