how do I form a prize certificate?

I am trying to create a Prize certificate for a small office, not quiet sure
what to do

You might start by looking at the certificate templates at
http://office.microsoft.com/templates/default.aspx?

It would probably be easier in Publisher, where you can do each line
independently.
