how do I find the check box field?

In Word 2003 or earlier, it's on the Forms toolbar.

In Word 2007, first go to Office button > Word Options > Popular and enable
the Developer tab on the ribbon. Then, on that tab, it's in the dropdown
from the Legacy Tools button.
