If you are using the "Controls" panel on the Developer tab, the controls
shown there all insert the new "Content Controls". For some reason no
"Checkbox control" was included in the content controls. You may find info.

Othersise, if you mouse over the 4th icon from the left on the bottom row
(with a folder with a couple of tools on top of it) you should see "Legacy
Tools". Click there, and you should see "Legacy Forms", which is probably
what you need if you were familiar with the "Online forms" in Word 2003 and
earlier, and ActiveX Controls, which probably /isn't/ what you need. In the
Legacy Forms, the checkbox control button should be second from the left.
If you want to use the legacy forms, you may need to backtrack a bit if you
already started using Content controls.
