How do I create an Index with more than two levels

In Word 2003, I have created an index. I have some topics that fall under
three headings, but I can not mark the document text to show up beyond two
deep.

Example:

Fruit
Apples
Banana
Blueberry
Strawberries


What I want to create
--F --
Fruit
Autumn
Apples
Spring
Strawberries
Summer
Blueberry
Winter
Banana

How can I mark the document's text to indent under two levels?

Add an additional colon in the XE field: { XE "Fruit:Autumn:Apples" }. You
can do this in the Mark Index Entry dialog by just typing "Autumn:Apples" in
the Subentry box.
