How do I add Radial Button activated Rating scripts for Access

I am trying to add a Field in a Table that will generated a number for that
field between 0 and 4. The purpose of this field is to create a Student
Feedback Form to rate a block of instruction.

I need to use a radial button selection process from "Poor" to "Excellent"
as Labels on the Form the students will fill out. Once a selection is made it
will place a value of "0" to "4" in the Field within each Record.

What can anyone do to assist me?
 
Add an option group to your forum. The wizard will lead you through it.
Set the option group 'control source' to the table column name.
