How do I add active radio buttons to my word document (word 2003)

All I wish to do is create a list of topics and allow users to check boxes
like in doing a survey.
 
Try using a 2 column table and in turn on the Forms toolbar from the Menu.
Insert checkbox formfields in left column and the Question in the 2nd
column.

Go Tools Protect Document Editing Restrictions (check box) and select
Filling in Forms only then Yes Start Enforcing Protection.

Tab and you will go to first checkbox and click it will mark as an "x".

You should be able to work from this - there are other ways that include
macros and userforms.

Do not add a password (unless you write it down as well).
