How can I show a continuous form in a report?

I have a continuous form in a subform, showing types of medication that a
patient is receiving. When I include these fields in a report, it only shows
the first record. How can I show all the records in the report? The report
is based on a parameter query requesting the patient id number, as it forms
the basis for a letter to the patients GP.
