After creating your template, choose Save As instead of Save. After keying
in your filename, select "Save as Type - Document template". This has a .dot
extension.

please help

In the File > Save As dialog, change the "Save as type" dropdown to
"Document template (*.dot)".

Word will automatically change the file extension to .dot and set the
current folder to your User Templates folder. You can change the part
of the name before the extension if you like. After you save it, you
can use the File > New command to make new documents based on the
template.

Save As > Word Document Template (type of file in the dialog box)

Unless you are talking about a form for people to fill in, in which case you
may want what Word calls an "online form." Check this in help. For more
about online forms, follow the links at

Chapman's series of articles. You may also want to look at

Hope this helps,
