How can I get a blank entry form to show when I open the database.

I have created a database that users will enter data using an entry form. I
want a blank form to show up each time a different user wants to add a
record. I don't want the record that they entered viewable to other users.
 
Need some clarification. First, will this form be used only to enter new
data, or will it also be used to view or modify existing records?
Then, you say you don't want a record entered by a user to be viewable to
all other users. Do you mean never, ever? As in any user can only ever see
the records they created?

If so, is there any mixing of this information that, perhaps, another
specific user or group of users who can see all the data? This is an unusual
request, but perhaps I am not understanding. Please give me a little more
detail.
