help with auto fill

i have a table "Patients" which i enter info thru my main form .I enter
patients details and have fields like"Patient,personal no, Diagnosis
,consultant, hospital etc" some patients we send for overseas treatment more
than once .i have to have a record for each visit.

what i need is .when i enter any patient(name) which is already got previous
record(and this is the 2nd or 3rd etc.) to auto fill the other fields "
Personal no , diagnosis, etc" with data from prev record for the same
patient. is this possible in access.
