Header Showing Randomly

I have a couple of problems I need assistance with. First, my header shows
properly on all pages but two. One of them is my coversheet which I do not
want it on. When I remove it from the coversheet, it disappears from Page 4.
I need it on page 4.

See if we can solve this first. Thanks in advance.
 
What did you do to "remove" It? (You can never actually remove a
header after it's been added to a document, but you can empty out all
the content so it doesn't show.)

Is p. 4 the start of a new section? Go to its header and on the header
toolbar uncheck "Same as Previous." Then fix it back to what it was
supposed to be.
 
Each page is showing up as a different section. (I did not realize that
before.) They are all (with the excpetion of the cover) part of one document.
The cover is well, just the cover.
 
If there's a good reason for making each page a new section, then
you'll have to go to each section's header and be sure that "Same as
Previous" _is_ turned on.

But it seems unlikely that that's the case, so it would be good to
remove as many section breaks as possible. You can start a new page
with Insert > Page Break.

If the cover is in the same document, then it's Section 1. (If it's a
separate document, you needn't mention it!)
