Header not appearing

When clicking on "View, Header" the header format box does not appear.
Double-clicking on where the header should be does not appear. I never see
the broken dotted lines where the header should be.
 
Make sure that you are not using the Hide Whitespace option and that you
haven't set the header Margin to zero.
