Good documentation practices

What types of documentation practices do you follow as an Access developer?
Are there good references on the subject?

I have developed a system throughout the complete development lifecycle and
I wanted to make sure I provide satisfactory documentation for whomever uses
my system in the future. I am interested in both user and programmer
documentation with programmer documentation as the priority.

Currently I have a Systems Requirements doc, a data dictionary, and the
relational diagram that SQL Server provides.

I am not entirely sure what you are referring to when you say "practices".
For a database I wrote an introductory user guide (menu's, navigation,
entering records and such like) and a technical document detailing the query
information in flowcharts.

Nothing beyond that.

By "practices" I mean, standard documentation that you maintain for your
systems, exactly what you mentioned. Going further, how do you document your
menus, navigation, etc? Do you take screen shots and use them in a word
document with text at the bottom of each screen shot? What does a query
flowchart look like? Thanks.
