FP 2003 Dynamic Web Templates

HELP (please)

Question 1:

What causes Dynamic Web Templates to become unlinked to htm pages?

Twice I have had this happen and it is a nightmare relinking.

Question 2
----------
My template has only three editable regions: left, right, middle.

Is there an easy way to relink all my unlinked pages so that the data
goes back to the correct 3 regions? I get a prompt to pick just one
region to stuff all the data for each page (making rearranging
necessary.

Question 3

The unlinking happened when I restarted FP 2003 and an old template
(from a few days back -- several sessions ago) suddenly reappeared even
though I had deleted it ages ago. Thoughts?

Just to add to the mysterious appearing file. It says created Jan 5th.
Modified December 31st. Bizarre.
 
Always modify the site in a disk-based Web or Windows SharePoint Services,
then publish it wherever you need.

Don't make changes on a FrontPage Server Extension site and then publish (or
synchronize) them back.

Do either of these remind you of something you did lately?

If you have "older" versions showing up you may have corrupt FP META files
1) In Windows Explorer Find and delete all FP Temp Files
Usually in C:\WINDOWS\Temp and %USERPROFILE%\Local Settings\Temp
- in FP 2003 you need to look in the super hidden folder
%USERPROFILE%\Local Settings\Temporary Internet Files\FrontPageTempDir
2) In Windows Explorer Find and delete all hidden *.web files
Usually in %USERPROFILE%\Application Data\Microsoft\Web Server Extensions\Cache
