Forms

Wow, that's a broad topic, Noora! The smartypants answer would be to go to
the Forms tab and click the New button and choose Design View from the
list. But it would be mean to leave you hanging like that! LOL.

I would suggest getting yourself a good book about Access. There are
several good ones available, probably at your local bookstore or favorite
online retailer. I am personally fond of the Sybex ones by Getz and
Litwin, but that's just me.

If you are familiar with Access and Form design, but just want to bypass the
Wizard, select Forms from the Object List & double-click the Icon at the top
of the list labeled "Create orm in Design view". You will need to use the
Form's Record Source property to specify what Table or Query the Form is to
be associated with.

Otherwise, Matt's suggestion is quite appropriate.
