FOrms question

I am trying to devise a form for our web that will submit a service request
via email. WHat is the best way to do this? I use Front page 03 on one
computer and Sharpoint Designer 07 on another, I have infopath 07 on both,
and we do not use an SQL server.

The questions will be typical name, phone email address, but I want to add a
few fields that will start with - name the problem A or B, if A the response
will generate a field will give a drop down list of requests, if Request = B
this will generate a drop down list of a diff set of problems, I have done
these lists in excel, but how to do in what program and make it a request
form on the web?
