forms help!!!

Word Wiz

I have created a form and tested it by sending it to a user to fill in. I
locked the form and sent it. None of the users including my self can update
the form by using the check boxes or text box. And now the forms tool bars
are not active when I open the form so I can not unlock or edit. Also when I
open the .dot form it creates a word copy not sure why.
 
The document could be in Design Mode. You can exit that mode via the button
on the Control Toolbox toolbar.
