Forms: E-Mail Notification to Two or More Recipients

I would like an e-mail notification from a FORM submission to be sent to two
or more recipients.

Everything works right when I send the e-mail notification to just one
recipient, but I cannot get it to work if I specify two different
recipients. I have tried the following syntaxes:

separated by a space
separated by a semicolon (only)
separated by a semicolon and a space
separated by a comma (only)
separated by a comma and a space
enclosed in quotation marks, separated by a space
enclosed in quotation marks, separated by a comma
enclosed in quotation marks, separated by a comma and a space
enclosed in quotation marks, separated by a semicolon
enclosed in quotation marks, separated by a semicolon and a space

When I try these, instead of getting notifications to both addresses, I get
none at all.

Is there some way I can have the FORM e-mail notification sent to two or
more addresses?
