Forms disappeared from Action menu.

Hi guys!

I had many forms under "Action" menu, but today i cannot find any forms
under "Action" menu, but when i going to "inbox" folder properties, i can see
all my forms under "Forms" box.
How can i get back all these forms under my "Action" menu?

Thank you!
 
Additional info is that when i create new form, Outlook give: Mapi error.
Hope it helps to resolve issue.
