Forms; Adding reminder dates to a form

Hello, i have designed a form in my contacts for the staff that work in our
company. This includes the general information (ie name, adress) and also one
for Renumeration (hourly rate, weekend rate, holiday pay, number of sick days
owed, and remaining, car allowance, etc). I would like to add a form where i
can add in spacific dates for induction and performance reviews. For example.
1 month after a new employee starts they have an informal review, then
another one at 2 months. A formal review at 3 months, then yearly ongoing
reviews. I would like this to be similar to the Birthday's and Anniversary
where you click on the arrow and the calendar drops down and it will remind
you when this is by poping up in Outlook. Please can someone help me with
this as i have looked everywhere and can not see how to add the drop down
calendar

You would have to write form code to create a new appointment item using the
start and end settings you want. A normal birthday reminder is set as an all
day event, so you may want to set that property.

To specifically answer the dropdown calendar question, look to see if the
Microsoft Date and Time Picker control will do the trick for you.
