I want to set up an outline format (as default) that has the space between the number or bullet of an outlined document to only be two spaces - how do I do that?

See http://www.shaunakelly.com/word/numbering/OutlineNumbering.html and
http://www.ShaunaKelly.com/word/bullets/controlbullets.html. You'll probably
need to add at least one of the spaces as hard text and choose "space" as
the separator in the Numbering dialog.

This is a suboptimal way to set up Q and A styles because you have to change
levels to change bullets (you can, however, do this with
Alt+Shift+Right/Left Arrow). Instead, use Q and A as custom bullets on two
styles, Question and Answer. Set each style as the "Style for following
paragraph" of the other, and they will alternate. Alternatively, if answers
often have more than one paragraph, set Answer as the next style for
Question and assign a keyboard shortcut to the Question style.
