Form to assign joint tenant

I want to assign my wife as a joint tenant on my utility stock. I would like
a form that is specific to that endeaver. It would include space for the
stock name and number. And have the query for a medallion signature gaurantee
from a notary

I'm not sure that you are talking about the Microsoft Access database
product. Your question may be better answered in a newsgroup appropriate to
your problem. This newsgroup is dedicated to the Microsoft Access databases.
