form not showing up

Help, I created a form in outlook that allows me to enter information then
send it to another person. The form shows up for me to input but when I send
it the person on the other end see's a blank email. How do I get the form to
show up in the email. I have looked and read until i'm half blind.
 
Are you both working in the same organization and using Exchange as your mail server? Where did you publish the form? Did you click the Edit Read Page button to create a read layout? See http://www.outlookcode.com/d/sendform.htm
