form for data entry

I need to display all records on a single form for each person. The record
source is the following:

Nancy,
Using repeating fields like Date1, Date2, etc. is an inefficient and
ineffective way to use a relational database. You have already discovered
how difficult it is to create a data entry form for that kind of data
structure. There are tons of other problems you will encounter if you keep
your table like that. You need to have more than one table. Can you describe
the business process you are trying to model?

Lynn,
The table is used to keep track of dates of phone calls, appointments, etc.
Sam has been thru 3 different levels of probation. Each level has dates for
when he went on probation, date for when letter notifying him was sent, date
for when he responded to the letter, date for when he is scheduled to meet
about the probation, etc. Since he has progressed from level1 to level3, we
want to see "at a glance" where he is in the process. We will enter a new
record for each level he is at. The main objective is to track him at a
glance. Hope I'm not too confusing with this explanation! Thanks for you
attention

Lynn,
And that is where I need suggestions! How do I create a single form that 1)
displays all of Sam's data;i.e. info for any/all levels of probation for Sam,
2) allows data entry for a new level, and 3) updates any/all levels
