Form file upload

I am trying to create a page where people can upload to a folder on the
website..However, I get an error "[FrontPage File Upload Component] ' when I
see the page where the form is... and also I tried putting in a file anyway
and it gave me a "Cannot run the FrontPage Server Extensions on this page:
"http://www.secondprovidence.org/trusteesubmit.htm"

not sure what to do here... i have the right extensions on the server

Is your site hosted on a Windows IIS web server?
Does your web host allow you to use this component?

I've tried the file upload feature with no success either.

Does the site have to be hosted on a Windows IIS web server? My site is
using an Apache Web Server.

I'm not sure it my web host allows this to be used.

Yes must be a Windows server w/ a host that supports it
