Is there a way that I can have a form duplicated whenever I add a new user to
a database without having to go in a create and format one each time?

I'm having trouble visualizing why you would need to create a new form
("have a form duplicated") for a new user.

If you'll describe a bit more about "what" you want to accomplish, rather
than "how" you've already decided to accomplish it ("have a form duplicated
whenever I add a new user"), the folks who read these posts may be able to
offer an alternate approach.

What I want to accomplish is that when I need to add a new user to the
database, rather than having to manually make a new form for that individual,
have a process that will do it automatically or mask the format that the
other forms have.

Why have a separate form for each user? Why not have one form and fill it
with
the data for the user of interest?

Sorry, but I'm still not getting it. It is very unusual to have a system
that needs a new form for each new user. I can't tell whether you and I are
using the word "form" differently, or if you have decided that the only way
to accomplish something (which I STILL don't understand) is to use a
separate form for each new user.
