Form auto opening when I open database

When I open a database (I used a template from online called CONTACTS, I
think), a form automatically opens that shows the first contact and data
about him/her. I altered this form and the record source it was pulling
from. I also DELETED the orginal form called ADDRESSES as I renamed it to be
FOOTBALL PLAYERS. But, now when I open the database it tried to open a form,
but gives me an error stating the ADDRESSES it refers to is not available or
something like that. How do I change whatever it is that automatically opens
this form to refer to the new source called FOOTBALL PLAYERS??

Tools>Startup
under "Display Form/Report" see if ADDRESSES is showing. If it is, change it
to the form you want.

If that's not it, you may have to do a search/replace of all code modules
for all instances of ADDRESSES

See if you have a macro named Autoexec with action to open the form.
