Form and Hyperlink

I created an order form using the Form Wizard. I have 2 colums or fields.
The first field is used to enter the quantity the person wants. The second
field has the description which was entered in the Text Box Properties under
the intial value. I would like the description to be a hyperlink so the
person will also be able to see what they are ordering. How do I change the
description in the intial value to a Hyperlink?

Why not put a thumbnailed image right next to the text field of the item? You could even have it open a small pop-up for the larger image using Spawn.
