Footnotes not at bottom of appropriate page

Two problems.

A few footnotes in a document not at the bottom of the page they're
referenced. Instead they're at the bottom of THE NEXT PAGE in the
document. Copy/paste doesn't work. Nor converting from footnotes to
endnotes. Also if you try to move the paragraph (in the document) of
the offending footnote to another location it does not renumber
automatically.

Please help ...
