footnotes disappeared.

some of my footnotes have disappeared. The sequence now starts wtih 5. Then
it goes randomly to 6, 8, 14, 19, and so on. It is no longer sequential. I
have tried all the usual options such as trying to start over with number 1,
changing from continuous to starting new.
 
Do you have Track Changes enabled?
