Fill-in field within a form

I want to have a 'form' with fill-in fields (that pop-up on opening),
check-boxes and some drop down lists. The only way I seem to be able to do
the check-boxes and drop-down lists is to use the form tools but that
prevents the fill-in fields from working. Does anyone know how to create a
form with both mail-merge and form tool features?
