File Defrag

When defragmenting a volume, I can't seem to get the
whole volume to completely defrag. There are still
numerous files left fragmented. ?
 
Which files remain fragmented and in how many pieces are they? What happens
if you boot into Safe Mode and defrag from there? Do you get better
results?
