Field not being submitted on a form

Howdy,

I have a form set up and everythign seems to work just fine except a couple of the fields aren't
being submitted when the form is sent in.

All the normal fields work fine but at the bottom I have s series of radio buttons in a "Where did
you hear about us" type question. The options are Internet site, TV, Friend, etc. Next to the
Internet Site radio button is a text box for them to type in the name of the internet site. Then
come the rest of the radio buttons.

Is that messing it up? The page is at:

The part that doesn't work is at the bottom of the page between the "Other Information" and the last
checkbox.

Any idea why?

In case it matters I'm using FP 2002 and the host DOES support FP extensions.
