I can't seem to get my fax services up and running. Used to be able to, but
after I reformatted, it's not working. I have gone to Add/Remove and and to
the Windows applications and checked the add fax services box. It starts the
process, goes halfway and then returns to the add/remove screen. When I go
back, nothing is checked and there certainly isn't a link to the fax console.
I even tried to use my original disk, but to no avail.

Event viewer ?
 
What am I looking for in Event Viewer. If I am looking for something to do
with the fax, I am not seeing any such event.

I just installed the FAX package (~4MB) via the Windows Components in Add/Remove.
These are the highlights.

C:\WINDOWS\system32\fxssvc.exe <--- New service in Services

"C:\WINDOWS\system32\fxsclnt.exe" /new <--- Fax control panel, called for first time.

C:\Documents and Settings\All Users\Application Data\Microsoft\Windows NT\MSFax

C:\Documents and Settings\All Users\Start Menu\Programs\Accessories\Communications\Fax

I can see separate files in the i386 folder, stuff that begins with
fxs. I didn't copy all those file names into this post - I'd assume
virtually everything with fxs as the first three letters, is being copied
somewhere.

I did get a few events in the Event Viewer, as I didn't have the modem set
up and so the Event Viewer got a few comments from the Fax software. After
setting up the modem, that stopped.

OK, by poking around the i386 folder, the FxsOcm.inf file seems to control
the Fax install. The file is stored in i386 as FxsOcm.in_ and needs to be
expanded, before opening it with Notepad.

The FxsOcm.inf says any debug output will be going to "FaxSetup.log".
And in typical Microsoft fashion, even though Fax was never installed
on this machine, that log has been filling since the install date (471KB!).
(Open that log with WordPad, as it is a mess.) Lots of useless cruft
in that file, so I hope if there is a real error listed, you'll be able
to find it near the end of the file.
