.exe file association restore

All ".exe" files are opening in Notepad. How to restore the file assocation?
I've tried the fix on Doug Knox's page mentioned in a previous post, but no
luck. I may not be doing it correctly. Any ideas?
 
How did you merge the exe fix from Doug's site? Alternately, try fixing the association manually:
