I have a survey on paper with 26 questions with 3 response choices. How would I set this up in Excel?

You _could_ set up a questionnaire in Excel, but a better tool for the task
might be SurveyMonkey or a similar free service. And, if the SurveyMonkey
results aren't in your preferred form, you could download the raw data and
use Excel for analysis and charts.
