Error After Print Set-Up Functions

I have had a problem for some time. It occurs with two situations: 1) when I
do print set-up, to REPEAT the top 3 lines on multiple pages, and 2) when I
do print set-up on a simple spreadsheet of "email addresses".
Eventually, I contacted HP thinking it MIGHT be related to my Photosmart
All-in-One printer support function in some manner. HP eventually said their
technical support was working on the problem and would contact me when a
solution was available. After over 30 days, I did a F.U. email asking the
status and they conveniently lost my problem references. They dropped the
issue!
Anyone have any ideas? I am not sure it's a printer related issue?
