Empty 'To' fields

When sending emails out of the domain, the to,cc and bcc
fields are empty when viewed in sent items. The messages
do reach all of the recipients though.

Outlook 2003 SP1, upgraded from 2000
Exchange 2003 SP1

Using any POP in Outlook, or is all mail handled by Exchange?
If you access the mailbox in OWA, does all look well?
Does this happen to *all* Outlook users?
