Ebay feedback

I would like to set up a Feedback page in the way ebay does. That means that
a customer would leave a comment and choose one of these ratings : positive,
neutral or negative. And at the top of the comments, a table would summarize
the percent of positive feedback and the feedback score.
Could you guide me in doing this complicated form ?
I already inserted the Frontpage feeback form, which is simple.

You would not be able to do this with the FP Feedback form. You would require a customized
server-side scripted database driven solution which would be based on what is supported by your web
host.

Do you know some example of this solution on the web ? I worked with
databases in the past and I could work on it if I had some guidance at the
beginning.
