Does anyone have a template for customer satisfaction surveys?

I need a template to enter the results of a 20 question customer survey.
I've looked at the Microsoft templates and none of them are similar enough to
be able to easily modify them. The answers to the survey are multiple
choice, yes/no, short discussion and contact information. I would also like
to be able to print out summary reports reflecting the input. If you know of
something I might be able to modify, I would be appreciative. I am using
version 2003 and XP. Thanks.
