documentation tips requested

hey all,
does anyone have some good tips or references on writing good documentation
alongside the application code (not comments)? I do write stuff down but 6
months from now if i have to come back to that documentation it's a little
difficult to pick up where i left off...

You mean besides using Xml comments?
