Disappearing inbox message on reply: not the usual causes?

I just had an email message vanish after I replied. I think the
sequence was:

1. reply to email.
2. delete original. (I'm not sure I did this, but it's possible I
deleted the original while the reply was open but not saved as draft.)

The odd part was the original didn't go to the Deleted Items folder as
expected, it simply vanished -- as though I'd emptied the Deleted
Items folder.

I use Outlook 2002 SP2 and connect to an older version of Exchange
server.

I checked my views, rules, spam filter, etc and reviewed the one
commonly identified kb article:

Still no explanation. I see several similar queries in the newsgroup
on this topic, but the usual explanation is the kb article and I don't
see any f/u to indicate there was any other problem.

I am the kind of user that odd things happen to (a curse), so I can't
make much of this behavior unless someone else has experienced it.
Most unsettling though.

Thanks for replying, but as I'd noted in my original post I knew of
that cause.

However, I am obliged to say that the message did turn up -- this
wasn't a bug, it was human error (mine). So, no evidence of any
disappearing inbox message causes other than those referenced in the
kb.
