Designing a Data Entry Form with fields based on classification

I have a database that has the following fields, client name, document name
and document date. I need to create a data entry form containing a field
which assigns each client a particular classification then pops up or shows a
set of questions based on that classification. The questions then must be
answered along with a YES/No box, then put apart of the client's record. Can
anyone help me?

What have you done toward this end so far? You'll find the newsgroups can
be quite helpful if you post what you've already done and ask specific
question about how to make some feature work.

Or are you asking someone to do this for you?
