I have found that MTWIRL.DLL file in infected (trojan
horse startpage.5.bt, it is in my c:\windows\system32
directory. I cannot delete it because the directory is
read only, and everytime I change the attributes it
doesn't seem to take any affect. How do I get rid of this
file?

go to command prompt.
use task manager to kill explorer.exe
delete .dll in ccommand prompt then use task manager again to restore your
